UNCPM 22013 - Evaluation of a pharmacist-led medication therapy management program for patients with diabetes and hypertension at an outpatient clinic in Malawi

The purpose of this study is to integrate pharmacists into the outpatient hypertension and diabetes clinics and analyze the impact of pharmacy services on blood pressure or HbA1c, hospital admissions history, and medication adherence. The information we learn by doing this study may help to improve hypertension and diabetes care in Malawi.
