Heart to Heart: BP Control Partners

The purpose of this research study is to see if home-based blood pressure monitoring when combined with team-based care led by your doctor's office can improve blood pressure levels in comparison to usual patterns of care. The long-term hope is to reduce risks for bad outcomes such as stroke by improving blood pressure control.
